Grand River Community Health Centre (GRCHC) is launching a new Primary Care Access Clinic to support individuals in Brantford, Brant, and Norfolk who do not currently have a family doctor or nurse practitioner.  

The clinic offers non-urgent, comprehensive health care appointments to those without primary care providers. This initiative is led by the Interprofessional Primary Care Team, established in 2024 through funding provided by Ontario Health.  This clinic has limited capacity to roster clients.

“We recognize the growing need in our communities for accessible primary care,” said Lynda Kohler, Executive Director at GRCHC. “This clinic is designed to support those who have been underserved, offering a broad range of health services in a compassionate, team-based environment.”  

Available services include:

  • Well-baby and child appointments
  • Vaccinations and preventative health care
  • Cancer screenings
  • Sexual health services and pregnancy support
  • Smoking cessation support
  • Mental health counselling
  • Management of non-urgent or chronic conditions
